The steps to take
Stap 1
Together with your partner, you should make decisions based on the three following questions:
The first question is "Which program fits our situation the best?"
- Natural Conception (emphasizing curative treatment and relaxation - 28 days)
- IVF Optimum (a long-term curative treatment and relaxation, concluded by a form of Assisted Reproduction - 28 days)
- IVF Plus (a short-term curative treatment and relaxation, concluded by a form of Assisted Reproduction - 14 days)
- IVF Standard (only treatment with a form of Assisted Reproduction in a relaxing environment - 7 days or shorter)
The second question iis "Which form of Assisted Reproduction are we considering?"
- IVF with donated egg cells
- IVF with own egg cells
- Another form of Assisted Reproduction, such as AI or IUI.
The third question is "When do we want to get started?"
In all cases, your answers to the questions in our Questionnaire will be used to evaluate the chances of successful treatment. By sending the Questionnaire form to PregDesire, you make a first step to remote consultation.
Remote Consultation
The information you provide in the Questionnaire provides the basis for the specialists evaluate, which type of treatment is most suitable in your particular situation. We would contact you if additional information is needed. Next, you would receive a treatment plan proposal. It will inform you whether the desired treatment is appropriate and potentially successful.
- If you opt the Natural Conception program, the start date of the treatment can be determined by mutual agreement. In this case, further preliminary steps, such as medication or check ups, are not necessary.
- If you opt for an IVF program, you will receive a plan of treatment including a schedule and instruction for medication. A signed Approval form indicates your readiness to start the program.
Step 2: Preparation at home
Women who want to follow the Natural Conception program don't need to take medication. They can start with the treatment at any desired moment, in contrast to the IVF programs.
Women who menstruate and for whom IVF & Egg Cell Donation is a suitable method, need to disable their natural hormonal function, because of the negative influence of it on the process. One injection (GnRH-analogue: Triptoreline, Diphereline or Decapeptyl) lasts about 3 weeks. The application can be done by your own gynecologist. For the IVF Plus program, your own gynecologist will be directing your care until you arrive in the Czech Republic. For the IVF Optimum program, your treatment will be directed by the medical doctor at the Center. The result of an ultrasound examination will show whether or when the treatment with estrogen can start.
Women who don't menstruate and are going to follow an IVF-cycle with own egg cells don't need to disable their natural hormonal function. They can start immediately treatment with synthetic estrogen. For the IVF Standard or IVF Plus programs, your own gynecologist will be directing your care until your arrival in the Czech Republic. During the examinations, the doctor will check whether the endometrium is suitable for receiving and nidation of the embryo. For the IVF Optimum program, your treatment will be directed by the medical doctor at the Center.
Step 3: Introductory Interview
The content of this interview depends of the program you are choose to participate in.
- For the Natural Conception program, you get acquainted with the specialist at the Spa Resort, talk about whether any changes have occurred during the past weeks/days, and you will go over the daily routine of treatment procedures. During the course of the stay, you will also meet once with the specialist at the Center, with whom you will discuss the results of the spa treatment and the possibilities of an IVF-cycle in case a pregnancy does not occur within 1-1.5 years.
- For the IVF Optimum program, you get acquainted with the specialist at the Center, talk about whether any changes occurred during the past weeks/days, the current course of preparation at home, the chances of successful treatment and the schedule of consultations preceding the IVF-cycle in the last week. An interview with the specialist of the Spa Resort takes place as well, during which you get discuss the daily routine of treatment procedures and make appointments for weekly check ups.
- For the IVF Plus program, you get acquainted with the specialist at the Center, talk about whether any changes have occurred during the past weeks/days, the current course of preparation at home, the chances of success, and the course of the IVF-cycle during the last week. A gynecological examination takes place as well. Furthermore, you will have an introductory interview with the specialist at the Spa Resort and discuss the daily routine of treatment procedures during the first week.
- For the IVF Standard program, an introductory interview will not take place with the specialist at the Spa Resort, because a curative treatment is not relevant. You You will have a visit with the specialist at the Centre, and discuss all aspects of the IVF-cycle. Included is a necessary gynecological examination.
Step 4: Preparation in the Center
During the implementation of the IVF cycles the Center has to make specific preparations.
- Donor choice
In case of IVF & Egg Cell Donation, a donor will be selected who features the same looks as you as much as possible. We try to choose donors of approximately the same height, eye color, hair color and the same blood type. However, a 100% match is not possible in all cases. - Medical preparation
During the IVF & Egg Cell Donation, the physical processes of both, the donor and the receiver are fine tuned. They need to be synchronized by application of the estrogen hormone. A regular check of the endometrium is performed, which serves as the basis for a possible adjustment of hormonal doses. As soon as the synchronization and stimulation is complete for both, the donor and the receiver, the procedure of egg cell gaining - fertilization - embryo transfer can begin. - The sperms
Making the sperms suitable for fertilization is part of the preparation. - Prior to the gaining of sperms a blood test has to be performed (HIVm HBsAg, HCV, TPHA) not longer than 7 days before cryopreservation.
- For fertilization, either fresh sperms or sperms gained by the MESA/TESA method can be used in case the sperms are not suitable for a successful fertilization. MESA/TESE obtains sperms by surgery. By the MESA method, sperms are sucked from the epididymis. By the TESE method, a piece of tissue is taken from the testicle, in which sperms are located. The surgery is carried out in the clinic under light anesthesia.
- Sperms of good quality, which will not be used for this attempt, can be frozen can be frozen utilizing our Cryopreservation Program. The frozen sperms can be used for a possible second or third attempt. For those attempts, sperms do not need to be gained again. It is up to you to decide whether or not you use the Cryopreservation Program.
Step 5: Egg Cell Gaining - Fertilization - Embryo Transfer
- Day 1
Once the egg cells have been ripened in the donor's ovary (or in your own ovaries when you follow the IVF-cycle with own egg cells), gaining takes place by a puncture. The gained egg cells are fertilized by the partner's sperms. On the same day, the treatment with pregnancy hormones (Utrogestan or Crinone) begins to make the endometrium prepared for nidation.
- Day 3 - 6
Within a few days after gaining the egg cells and the fertilization in the lab, you will be informed about the number of embryos and their quality. Meanwhile, the embryos stay in the laboratory to carefully watch their development. Just before the transfer, Assisted Hatching is performed to ensure a good nidation of the embryo. - Embryos of good quality which will not be used at this attempt can be frozen if you choose to participate in the Cryopreservation Program. The frozen embryos can be used for a possible second or third attempt. At these attempts, it is not necessary to obtain neither egg cells nor sperms again. It is up you to decide whether or not you subscribe to the Cryopreservation Program.
- All preparations lead in the end to the most exiting moment, the Transfer, in which two or three embryos are inserted in your womb. It is the simplest procedure of the whole treatment until now, but for you, it is the one that is the most emotional of all.

Day 7
After the last examination to see if everything is all right, you can head for home. The next 14 days are filled with taking pregnancy hormones (Estrogen and Gestagen), and ... waiting. Try to to make the waiting period as relaxing as possible.
- After the Treatment
On the 21st day after the embryo transfer, the most important moment has come: you can take a pregnancy test with your morning urine (hCG). - If the test result is positive, you continue administering hormones for the next two months. You are in the family way, just as all mothers who got pregnant in a natural way!
- If the test result is negative, a blood test performed by your gynecologist will indicate whether or not pregnancy was reached or the therapy was unfortunately not successful.
- In the latter case, you can decide to repeat the IVF & Donation Program when the time is right for you. If there are cryopreserved embryos, it is possible to skip some phases of the procedure and shorten the stay in the Czech Republic.
